眼瞼型春季カタルの冷凍療法

Treatment of severe vernal conjunctivitis by cryosurgery

緒　言

　春季カタルの原因は，不明であるが高温または光線のような物理的要素が誘発因子となつて，外因性アレルゲンまたは自己免疫によつて結膜に過敏性が発生し，アトピー性体質，内分泌異常やワゴトニーのある若年者に好発する疾患であるとDuke-Elder^1）はのべ，多くの人々もこのように理解している。本症の根治療法は未だないが，発症再燃時に対症療法を行なつて自然治癒を待つのが通例である。対症療法に最も効果を発揮するのはステロイド剤の投与であるが，時に治療に抵抗し，角膜障害を併発したり経過が遷延する難治性のものが見られる。これらに対しては，ステロイド全身投与^1），β線療法^2,3），結膜瞼板の切除術^4）など種々な治療が試みられているが，副作用の点から長時間の治療には適さないものもある。ところで，Amoils^5）は1975年，眼科における冷凍手術の応用として，春季カタルに冷凍療法を試みその成果を，また本邦では古く1924年，秋谷^6）が春季加答児に対し雪状炭酸を貼用してその効果をのべているが，追試報告は内外を通じてない。今回われわれはすでに種々な治療が行なわれたにもかかわらず角膜障害を併発した難治性の眼瞼型春季カタル8例に冷凍療法を試み二，三の知見を得たのでここに報告する。

We treated 8 cases of severe vernal conjuncti-vitis (palpebral type) by cryosurgery using Cryo-method M-C 1000 apparatus. All the 8 cases had failed to favorably respond to instillation of conventional corticosteroid solution and all cases had developed persistent corneal lesions. A 4-mm glaucoma cryoprobe at the temperature of about -7℃ was applied to the affected irregular sur-face of giant papillae in the upper palpebral conjunctiva. Application times of about 10 seconds were used with freeze-thaw cycles repeated twice or thrice. The entire affected area was treatedin one sitting under surface anesthesia.
