眼サルコイドーシスの網膜血管変化について—走行異常および血管瘤形成

Retinal vascular changes of ocular sarcoidosis

緒　言

　眼サルコイドーシスの所見とくに網膜血管の変化については，わが国でも多数の報告がみられ本症の重要な所見の一つとされている。著者らも現在，経過を観察している35例の眼サルコイドーシスの患者のうち17例に網膜血管炎その他の血管病変を認めている。そのうち6例に網膜血管周囲炎に合併して起こつたと思われる特徴のある限局性の血管の走行異常を認めた。この変化は，これまで本症の診断に役立つ特有な所見といわれる網膜静脈周囲炎や網膜の浸出斑につけ加えられるべき所見ではないかと考えたので，その変化について報告し，乳頭表面に血管瘤形成をみた比較的まれな1症例についても併せて報告してみたい。

From a review of 35 cases with established diagnosis of ocular sarcoidosis, we detected 6 cases with abnormal kinkings of ocular venules and one case with an aneurysm-simulating change on the optic disc. The abnormal kinkings disappeared during the course of the disease and were thought to be due to focal displacement of the vessel by perivascular nodular growth. The aneurysm-like change on the optic disc involved the artery and disappeared in a few months.

These vascular lesions have not been hitherto described to the best of our knowledge. They can be added as new characteristic fundus manifestations of ocular sarcoidosis.
