Behçet病の抗凝固療法について

The effect of anticoagulant therapy on Behçet's disease

緒　言

　Behçet病患者のブドウ膜炎発作のmechanismは未だ不明であるが，Chajek^1）はフィブリノゲン量の増加，血液凝固第Ⅷ因子活性の著明な亢進を報告した。わが国でも，中山^2）が血小板粘着能および血小板のADP凝集能の亢進している症例の多いことを報告している。

　一方，Behçet病患者では，血沈値の促進している症例の多いこと^3）はよく知られているが，我我は，この血沈促進因子として，フィブリノゲンが最も高い正の相関関係にあることを既に報告した^4）。そしてさらに血液凝固能については，血小板凝集能の亢進，活性化部分トロンボプラスチン時間（APTT）の短縮傾向，フィブリノゲン量の増加，血液凝固第Ⅷ因子（AHF）活性の増加が見られるなど，一般に凝固亢進状態にあり，またhemocoagulaseの筋注により症状の悪化すること^5）も知つた。さらに，眼症状の寛解時と発作時についてみると，特に発作時にAHF活性の亢進が著明で，同時に測定したAHF様抗原量はさらに増量しており，眼局所における血栓形成傾向を裏づけていることも報告した^6）。

The antithrombin Ⅲ, which is one of the phy-siological inhibitors to the activated factor X, was decreased on many occasions in the stages both of exacerbated ocular inflammatory manifestation (attack) and remission, and the level of α2-macro-globulin was within normal limits. While the α1-antitrypsin, which is one of the physiological anti-plasmins was increased both in the stage of attack and in remission. On the other hand, the platelet life span (T 1/2) measured by the non-isotope method was apparently shortened in the stage of attack.
