副鼻腔手術後視力障害を来たした1症例

A case of loss of vision after radical operation for maxillary and ethmoidal sinusitis

　眼と副鼻腔とは，解剖学的に密接な関係があり，副鼻腔疾患によつて起される眼疾患としては，球後視神経炎を筆頭として枚挙にいとまが，ないがしかし普通，これらの副鼻腔炎による眼疾患は，副鼻腔手術に依つて治癒に向う事は日常我我が経験する所である。他方，副鼻腔手術に依つて起される眼障害も稀有とは云えず，現在まで数十例の報告がなされている。その障害を列挙すれば，眼瞼の浮腫状腫脹，皮下溢血，知覚異常，結膜下出血，結膜浮腫，涙嚢炎，鼻涙管狭窄，眼筋麻痺，眼球突出，眼球陥凹，視神経萎縮，視束陥裂，視束管内出血，網膜出血，中心動脈栓塞等が挙げられる。この内，視神経障害を起した症例としては，松岡^1），坂口^2），河原^3），山末^4），増田^5），竹内^6），志熊^7），鈴木^8），広瀬^9），栗崎^10）等に依つて報告されている。

　最近，教室に於ても，副鼻腔手術後視力障害を来たした1例を経験したので報告する。

A patient, 21-year-old man, who complaind of loss of vision in his left eye was first examined in our clinic on May. 3. 1960. Two days prior to the first examination, he had undergone a radical operation for maxillary and ethmoidal sinusitis, and on the following day he noticed the complete loss of vision.

The light perception appeared on the 10th postoperative day and then the visual acuity gradually recovered to 0.3. After 55 days, the defect of the nasal side in the visual field and slight pallor of the temporal part of the optic disc were presented.
