視力障害を示した副鼻腔原発アスペルギルス症

A case of aspergillosis of paranasal sinus with visual impairment

　視力障害で発症した副鼻腔原発のアスペルギルス症の1例について報告した。症例は23歳女性で左眼の急激な視力低下をきたし，ついでけいれん発作も引き起こした。神経放射線学的に上顎洞から蝶形骨洞，篩骨洞にかけての占拠性病変がみられた。手術により副鼻腔を満たしている膿汁をドレナージし，視神経を減圧した。組織病理検査によりアスペルギルスを検出し，アスペルギルス症と診断した。抗真菌剤を投与することにより神経症状は軽快した。視神経障害を伴う副鼻腔炎の所見を得れば，アスペルギルス症も念頭におき早急な治療が必要と思われる。

A 23-year-old female developed acute visual impairment in her left eye followed by convulsive seazures. Radiographic studies showed pus material filling the maxillary, sphenoidal and ethmoidal sinus. Aspergillosis was diagnosed after drainage. The condition subsided following decompression to the optic nerve by drainage and by systemic antifungal agents. Aspergillosis is a possibility when optic nerve impair-ment is associated with parasinusitis.
