内眼炎を合併した眼窩炎性偽腫瘍の1例

A case of orbital pseudotumor with intraocular inflammation

要約　目的：両眼に内眼炎があり，眼窩炎性偽腫瘍として治療した症例の報告。症例と所見：54歳女性が3か月前からの両眼の虹彩炎で受診した。1か月後に炎症が悪化した。シンチグラフィでガリウムが集積する腫瘤が両側の眼窩内にあった。悪性リンパ腫が疑われたが，眼窩と硝子体の生検では確認できず，眼窩炎性偽腫瘍による内眼炎が疑われた。副腎皮質ステロイドの点眼と全身投与ならびに硝子体手術で2か月後に内眼炎は鎮静化した。結論：臨床像は典型的な悪性リンパ腫でありながら確定診断に至らず，内眼炎を伴う眼窩炎性偽腫瘍とみなすべき症例がある。

Abstract.　Purpose：To report a case of bilateral endophthalmitis with orbital involvement simulating malignant lymphoma. Case and Findings：A 54-year-old woman presented with iridocyclitis in both eyes since 3 months before. A tumor was detected in both orbits with positive findings by galium scintigraphy. The findings were suggestive of malignant lymphoma. Biopsy of the orbit and vitreous gave negative findings. She was tentatively diagnosed with orbital pseudotumor with endophthalmitis. The intraocular inflammation subsided 2 months after vitrectomy and treatment with topical and systemic corticosteroid. Conclusion：Pseudotumor of the orbit may simulate malignant lymphoma when associated with intraocular inflammation.
