脳腫瘍を疑われた後副鼻洞炎による四分の一半盲症に就て

On the quadrantic scotoma caused by the posterior paranasal sinusitis.

緒言

　四分の一半盲症の原因について，今までは第二次視中枢である鳥距溝の障碍即ち後頭部の外傷等が主として考えられ，視束交叉部附近の病変は考えられなかつた。処が最近アメリカ学派は半交叉の状態について，従来の考え方の様に神経線維が正確に半交叉するのではなく複雑な経過を辿る事を発見した。この新しい半交叉の理論より視束交叉部附近の病変のみでも四分の一半盲症が起ると考える事が可能となつて来た。

　私は最近左側同名性下四分の一半盲症を呈し脳腫瘍を疑われた患者に後副鼻洞炎を発見し，その根治手術を行つた所急速に視野欠損が消失した甚だ興味ある症例を得たので茲に報告する。

A patient had presented the quadrantic scotoma in the left lower homonymous field and had been suspected as encephaloma. But the author found the posterior paranasal sinusitis in this case, and performed the intranasal sinus operation. As the result the patient was rapid in vanishing of scotoma.

The author concluded that this quadrantic scotoma was due to the upside pressure of the inflammatory edema, which arose in the arachnoid around chiasma on account of the posterior paranasal sinusitis, upon the right pathway behind semidecussation.
