A 42-year-old female had been diagnosed as parasagittal tumor 6 years before. The surgically removed tumor was identified as meningioma. She now underwent subtotal removal of the tumor due to remission. After surgery, she developed congruous right lower quadrantanopia with a sharp straight borderline along the horizontal meridian. Magnetic resonance imaging with contrast enhancement showed residual tumor surrounded by tissue edema in the upper left occipal lobe. The abnormal lesion did not extend into the upper bank of the calcarine fissure, or Vl. This case illustrates that a quadrantic visual field defect may develop due to parietooccipital lesion sparing the V1 area of the calcarine fissure.
