（18）詐盲の研究—（その1）詐盲発見の新考察

The studies about visual simulation.:A new conception against the discovery of visual simulation.

I．緒言

　著者が東京労災病院で過去1ヵ年間労災患者の障碍等級の認定を行つて痛感したことは，1）詐盲患者が非常に多いこと。しかも大多数の眼科医が詐盲に無関心であること。しかしこのことは外傷眼の予後を正確に知るためにも，或は又障害等級の正確な査定をする上からも非常に重要なことと思われる。2）従来の詐盲検査法は0.1以下の視力を訴える様な高度の視力障害には余り成績がよくなかつた。しかし著者の病院の様な所では成る可く真の視力を出さねばならないので，この点今迄非常な困難に遭遇していた。3）従来の詐者検査法は一貫したデーターを基礎とした系統的分類及びそれらの優劣を討したものがなかつた。そこで患者は偏光フイルターを応用して新しい詐盲検査法を考案し，従来のものよりも優れた成績を得たので発表する。さらにこの偏光フィルター法（以後偏光法と略称する）をも含め，従来の詐旨検査法の中から適当なものを5種含めて，3）の目的のために，ここでは0.1以下の高度の視力障害を訴えるものと，0.1以上0.6以下の視力障害を訴えるものに分けて，各々の検査法の成績を比較検討した。そうして0.1以下の場台は何れの方法が有効であり，0.1以上の場合には何れの方法が好いかを系統立ててみた。

　尚今度は主として眼科の所見，症状のみを主として判定したので，外傷性神経症或は外傷性ヒステリーをも含めた広義の詐盲を対称にした。

The author has noticed by visual conclusion in Tokyo Rosai Hospital that there were many patients who complained of visual simulation and the great majority of ophthalamologists were unconcerned about them and the usual tests were not useful for the high grade visual disturbance.

The new test was originated by the author applying the polar filter. Especially this test was indispensable for the high grade visual disturbance under 0.1.

Namely, 92% of visual conclusion were visual simulation and their visual increasing rate were 34%. 66% of them could obtaine the highest vision by the polar filter.
