Bowen病の1例

A Case of Bowen's Disease

緒言

　1912年，ボストンの皮膚科医J．T．　Bowen^1）が2名の男子の臀部あるいは下肢の皮膚に，組織学上奇異な病像を呈する皮膚疾患を認め，その組織学的所見をprecancerous dyskeratosis，この特色を有する慢性疾患をprecancerous dermato—sisと名付けた。これより2年後パリの著名な皮膚科医J．　Darier^2）は，この疾患が明らかな独立疾患単位であることを確認し，ボーエン病3例のうち1例に悪性腫瘍の発生を観察した。そしてこれをdermatose précancéreuseと呼び，ボーエンの提唱した名称に臨床的な裏付けを提供した。

　ついで，1921年，Jessner^3）は口腔粘膜に発生したボーエン病を記載した。眼科領域におけるボーエン病の報告は，1942年のJ．S．　McGavic^4）のそれをもつて嚆矢とする。彼は，5例の角結膜におけるボーエン病の症例を呈示しているが，これはいずれも顕微鏡標本を検索するうちに得られた診断であつて，臨床的には2例を見ているのみである。今回著者らはボーエン病と思われる症例1例に遭遇し，表層角膜移植を施行して良好な結果を得たのでそれについて報告する。

Paucity of reported cases of Bowen's disease justifies our presentation of the clinical picture, histopathological findings as well as therapeu-tic considerations of a 66-year-old female. This Precancerous condition, simulating conventional pterygium, developed at the temporal corneal limbus in the right eye. Profuse neovasculari-zation over the entire cornea and the conjunc-tiva constituted further remarkable findings.

Therapeutic lamellar keratoplasty was per-formed. Light- and electron microscopic inves-tigations confirmed the clinical diagnosis of Bo-wen's disease.
