A Brothers Case of Congenital Hereditary Corneal Dystrophy (Maumenee)

緒言

　角膜の先天性疾患に対する認識はわが国と特に白人種国との間には個体差あるいは検査設備の差などにより少なからずひらきがあつたように思われる。さらに諸外国においては特に角膜移植術の普及により稀少例のrecipientからの形態学的研究が急速に進歩し先天性角膜疾患の分類の上でも大きな変化をよぎなくされてきている。そのために従来異なつた呼称が与えられていた疾患が実は同一であつたりその逆であつたりすることも起こり比較的症例の少ないわが国で報告する場合ますます困惑させられる結果にもなる．本編で報告するCongenital Hereditary Corneal Dystrophy（以後CHCDと略）もその例にもれず諸外国で使用されたいくつかの呼称を列挙すると第1表のごとくなり，これらは1961年MaumeneeがレビューしたCHCH^1）と同一もしくは近似した疾患と考えられる。わが国でもKomoto^2）以来5編のあくまで類似疾患と考えられる報告^3〜6）をみるが，CHCDの認識にたつた検討は本編をもつて嚆矢とする。今回の報告ではCHCDの分類上の位置，鑑別診断，最近の特に形態学的所見からの成因論に対し主に文献的に言及する。

Since the congenital hereditary corneal dys-trophy was reviewed in 1960 by Maumenee, the term referred to a bilateral symmetrical diffuse opacification of the cornea, with edema but without vascularization, present from an early age and acompanied by marked impair-ment of vision (Pearce, 1969). The purpose of this paper is to present a family case of this disorder and to discussand review the similar conditions reported in Japan.

A family wherein two brothers were affected with this disorder had been described. Proposi-tus was the 13-year-old boy, whose parents noted corneal opacity at his birth which had been stationary.
