眼瞼に原発したlymphoblastic lymphosarcomaの1例

A Case of Primary Lymphoblastic Lymphosarcoma Occuring in the Eyelid

緒言

　悪性リンパ腫が眼瞼に発生することは稀である。眼およびその付属器のリンパ腫に関する諸家の報告^{2,3,8,9,13,19,23）}をみると，眼瞼は悪性リンパ腫の発生部位としては，より稀な部位に属している。また母集団を眼瞼の全悪性腫瘍にとつた統計^1）でも，眼瞼の悪性リンパ腫はわずかその0.1％を占めるにすぎない。

　著者らは眼瞼に原発したと考えられるlym—phoblastic lymphosarcomaの症例1例を経験した。眼瞼に原発したlymphoblastic lympho—sarcomaは文献上，Jain^5）の報告した1例と金子，谷島^22）による臨床報告の中にみられる1例の計2例しかない。極めて稀なものと考えられる症例をここに報告し，眼瞼に原発したlymphobla—stic lymphosarcomaの臨床経過について考察を加え，また，本症例の組織像を，従来広く行なわれてきたRappaportの分類よりは大きく変化した最近の悪性リンパ腫の分類^6,7,15）に，関連づけて言及したい。

A 64-year-old male developed lymphoblastic lymphosarcoma originating in the right upper eyelid without apparent systemic involvement. During 6 months following the initial onset, the tumor extended into the lateral canthal region and the lateral half of the right lower eyelid. We treated the case with surgical resection of the tumor and chemotherapy. No local or sys-temic recurrence has been observed during follow-up period of over 7 months.
