眼瞼の大欠損例5例に対する眼瞼再建手術

Reconstruction of Large Defect of the Eyelid in Five Cases

緒言

　眼瞼は，その特殊な構造と機能のため，その欠損の再建にあたつては，いくつかの固有の問題があり，眼科形成外科的に重要な位置を占めている。特に眼瞼の全層大欠損に対する形成手術は，眼瞼腫瘍の摘出後眼球を保護しかつ眼瞼部の美容目的のためには欠くことのできない手術である。したがつてその再建法としていろいろの技術が考えられてきた。それらを要約すると次の三つの術式に大別できる。

　1．Sliding flap法：上下眼瞼の何れか残つた方を，失われた方の再建に供する術式であつて，Hughes^6），Fox^4），Culter　＆　Beard^2），Smith^14）らによる各種の変法があり，よく用いられる。

We performed reconstructive surgery to re-medy extensive defect of the eyelid resulting from surgical removal of malignant tumors in five cases. In one case, the defect involving two-thirds of the lower eyelid could be successfully reconstructed by cheek rotation flap technique. In three cases, the defect involved from one half to the whole upper eyelid. Satisfactory results were achieved by applying the switch flap method of Mustardé. In one case, the defect involved the whole upper eyelid, lateral canthus and twothirds of the lower eyelid.
