眼球保存療法による網膜芽細胞腫の治験

Experience in Treatment of Retinoblastoma by Non-Enucleative Procedures

I．緒言

　好発年齢が2歳以下といわれる網膜芽細胞腫が，視機能のみならず生命の予後に及ぼす影響は，早期に発見治療されるか否かによつて大きく左右される。しかし，乳幼児眼底検診が普及していない今日，網膜芽細胞腫と診断される時にはすでにかなり進行していて，重症側眼の摘出を余儀なくされるのがほとんどといつてよい。したがつて，本症が15％以上の頻度で両側性に出現することから，われわれは軽症側眼の保存に全力を投じなければならないし，たとえ片側性であつても，他眼の定期検査は十分にくり返される必要がある。

　われわれは，眼球摘出後の病理組織学的検査によって，分化型網膜芽細胞腫と診断された3症例の軽症側眼に対して，Zeiss光凝固装置による光凝固を主とした保存的療法を経験した。このうち第1例は，百々，平田^16）（1964）が報告したもので，ここにその後の経過を報じ，2症例を追加する。

In 3 cases of bilateral retinoblastoma, non-enucleative procedures (light coagulation, dia-thermic coagulation and use of anticarcinoge-nic preparations) were administered to the eye with the milder degree of affliction. The at-tempt proved successful in not only destroying the tumor, but at the same time it was possi-ble to restore visual acuity to practically that of normal.
