未熟児網膜症に対する片眼凝固例の臨床経過について

Clinical Observations of Photocoagulation on Retinopathy of Prematurity

緒言

　未熟児網膜症に対する光凝固法は，すでに確立された治療法として広く行なわれているが，実施の時期，方法については現在でも研究者間で相違がみられるようである。本症には，insidious ty—pe，いわゆるrush type，さらに両者の混合型ともいうべき病型があり，それぞれの型によつて治療法も異なる。とくに，insidious typeでは自然寛解がきわめて多いことは周知の事実である，いずれにしても，活動期に続く瘢痕期の病変と，数年後から起こる晩発性合併症があり，この合併症まで考慮すると凝固の時期，方法についてはなお今後の研究にまたねばならない問題が残されている。著者らは，もつとも多いinsidioustypeの治療について5年間の経験に基づき，片眼を光凝固してその臨床経過を非凝固眼とともに詳細に比較検討したのでここに報告する。

Since June 1972, unilateral photocoagulation therapy with the light coagulator MF 1000 (MIRA, Inc.) has been undertaken on 25 in-fants with retinopathy of prematurity. Our criteria for photocoagulation therapy were aggravating changes in the fundus appearance in the middle of stage 3, such as tortuosity and dilatation of the vessels from the optic disc and the wide avascular zone at the peripheral retina. As the non-treated eye showed less progression of retinopathy than that of the treated eye in 13 out of 25 cases, accurate evaluation of the treatment was discussed concerning the remaining 12 cases.
