An improved surgical approach is necessary for cases of retinal detachment with highly vaulted retina and scanty tendency of reattach-ment, as usual methods of detachment surge-ry, as simple retinopexy through perforating diathermy and even scleral shortening opera-tions, prove often to be inadequate or inefficient.

The present authors are performing, for these instances, the scleral buckling procedures of Schepens with the use of silicone strips and tantalum clip. In the presence of massive vit-reous retraction, the techniqueis often supple-mented by the intravitreal injection of artificial aqueous humor.
