眼科領域におけるASL-O値およびその新しい測定の検討

Evaluation of the Method and Significance of ASL-O in Ophthalmology

I．まえがき

　眼科領域に限らず，一般にその病態と従来のASL-O値との関係が矛盾する疾病にしばしば遭遇することがある。すなわち同一眼科疾病においてもASL-Oの値がばらばらであり，これを治療の対照目的に入れるべきか困惑する場合が非常に多い。これには従来のASL-O値の感度の低さもその一因があろうと考えられる^1）。そしてその感度の低さの一因にはSL-Oの純度の問題があるであろうと考えられる。そこでこの数年来大国らはSL-Oの精製をすすめるとともに，新しいASL-O測定法として，ホルマリン処理感作血球凝集反応を応用した方法を考案し，これが従来の方法に比してどのような関係をもち，また感度のよさがあるかを検討してきた^2）3）。一方眼科疾病において，ASL-Oか高値を示す疾患，またそれに類似の疾患でASL-O値の高くないものなど種々病例をとりあげ，これらの患者の血清を対称として従来のTodd単位によるもの^12）と新しいHaemagglutination（HA）によるわれわれの方法とを比較検討し，いささか知見を得たので報告する。

A refined technique of evaluation of antistre-ptolysin-O titer (ASL-O) has been developed by extracting highly purified streptolysin-O from Streptococcus pyogenes and the use of formalin-trated erythrocytes. The authors' method ena-bled the titration of IgM (19 S) antibody fraction as against the hitherto current Todd's method which was mainly concerned with IgG ( 7 S) antibody. The authors' method also enabled the exact estimation of the antibody during the early stage of its formation. Values of less than 1, 280 are considered to lie within the normal range.
