糖尿病患者涙液の糖呈色反応

Glucose Detection in Tear Fluid in Diabetics

I．はじめに

　糖尿病患者の涙液糖量が正常者に比べてかなり高いことは，以前（昭38）徳田，村田^1）の報告したとおりである。しかし涙液の採取が容易でないところから，実際の臨床ではなんらとりあげるに至つていない。そこでテステープ，クリニスティックスなどによる糖呈色反応が，血糖値のある程度の上昇につれて確実に陽性に出るならば，高血糖の予知や糖尿病性こん睡の診断などで，その簡便性の点からかなりの実用性をもつことになる。しかしこの方面の報告は，Lewis^2）（1958）およびGasset^3）（1968）が，クリニスティックスを用いて行なつているだけで，わが国での報告は皆無である。LewisはいわゆるHyperglycemiaとして160mg／dl以上をとりあげ，その場合85％が陽性にでると述べており，Gassetは，糖負荷後の平均血糖値193mg／dlにさいして，96％（25／26例）が呈色反応陽性を示し，高血糖を間接的に知る方法として，この涙液の糖呈色反応が簡便かつ迅速という点ですぐれていることを強調している。

　われわれは最近糖尿病患者の集団検診を利用して，この涙液の糖呈色反応をテステープとクリニスティックスの両者を用いて検討した結果，さらに新しい二，三の知見を得たのでここに報告する。

Glucose detection test in the tear fluid was conducted with the combined use of Schirmer paper and glucose reagent strips (Testape or Clinistix). A total of 161 diabetic subjects were studied during an occasion of mass follow-up examination.

In cases with blood glucose level of less than 100mg/dl, positive glucose in the tear fluid wasdetected in 11% of the cases with Testape and in 57% with Clinistix. In cases with blood glu-cose level between 160 and 200 mg/dl, positive tear glucose was detected in 87. 5% with Test-ape and in 50% with Clinistix.
