瞳孔緊張症の自験6症例から見た本症の問題点

Six Cases of Pupillotonia

I．緒言

　瞳孔緊張症の存在は1812年Ware^1）の報告に始まるが，19世紀までは反射性瞳孔強直症（ArgyllRobertson）瞳孔の一異型であると信じられていた。1921年Behr^2）が瞳孔緊張症はArgyllRobertson瞳孔とは区別すべきことを強調した。1931年Adieが本症に腱反射消失を伴うことが多いことを強調し，後年本症はAdie氏症候群として，世に広く知られてきた。したがつて瞳孔緊張症とAdie氏症候群（瞳孔緊張症＋腱反射消失）とはまつたく同じものであつて，Adie氏症候群とは瞳孔緊張症の一型にすぎないわけである。にもかかわらず，両者が全く異なつたclini—cal entityと考えられている。私達はこの論文において自験の6症例を介して瞳孔緊張症の問題点を論じていきたい。

This paper reported 6 cases of the tonic pu-pil and discussed the problems of pupillotonia.

All the cases were female and the majority (5 out of 6) were in their twenties at the on-set of the condition. The tonic pupil was uni-lateral in all cases. The tendon reflexes in lower extremities were normal in three cases and abolished in the others. Therefore, arefle-xia does not necessarily occur in conjunction with the tonic pupil and Adie's syndrome should be regarded as a form of pupillotonia.

The Mecholyl tests were positive and Wa-ssermann's reactions negative in all cases.
