瞳孔緊張症における対光反応の分析

Tonic pupil and its autonomic nerve imbalance

　Open-loop赤外線電子瞳孔計を用いて，瞳孔緊張症3例の対光反応の結果を分析し，さらに低濃度ピロカルピンの点眼による瞳孔の大きさ（面積）の変動を求めたところ，次のような結論を得た。

　（1）瞳孔緊張症の対光反応は副交感神経刺激型（cholinergic）のパターンを呈した。これは自己の常在性のアセチルコリンおよび対光反応により放出されたアセチルコリンに対して過敏性を示しているものと考えた。

　（2）0.0313（2⁻⁵）％ピロカルピン2回点眼によって，2.5％メコリール点眼と同様に瞳孔緊張症の診断に応用できることを証明した。

　（3）瞳孔緊張症の場合もopen-loop光刺激下に対光反応を瞳孔面積でもってとらえることが必要であると強調した。

Three patients with unilateral tonic pupil were investigated for pupillary light reflex under open-loop photic stimulus.

Following 15 minutes dark adaptation, successive 5 pupillary light reflexes were measured alternative-ly on both eyes with adequate rest by the use of a new infrared videopupillogram. The videopupil-logram was developed by one of us (Utsumi) which could elicit the light reflex by the photie stimulus under open-loop condition. The effects of tonic pupils on each pupillary parameters (Fig.1) were obtained from the following formula:
