所謂ステロイド緑内障について

Steroid Glaucoma

　副腎皮質ホルモンは眼科領域において局所的にも全身的にも広範囲に使用され種々の疾患に著効を奏する事は今更いう迄もない。元来ステロイド剤は続発性緑内障には眼圧下降に作用するものと一般にみとめられているがステロイドの眼注により急性続発性緑内障をおこした事を池田氏^1）浅井氏^2）が報告している。而し長期間使用する場合に慢性緑内障を合併する事が取上げられ1962年H．Goldman^3）がこれをCortisone glaucomaとして報告し更に本邦においても岩田氏^4）は春季カタルと緑内障合併を，又小川氏^5），清水氏^6）はこれらステロイド剤投与によつて起つた例を報告しCortisoneのみならず他のCorticosteroidにても発生する点からこれをSteroid Glaucomaと呼ぶのが妥当であろうと報告している。我々も今回2〜3の疾患についてCorticosteroid治療中に緑内障を併発した例に遭遇したのでここに報告したい。

Nine case are reported in which pathologi-cal increase of intraocular pressure occurred during prolonged topical corticosteroid the-rapy to ruch eye conditions as subconjuncti-vitis, vernal conjunctival catarrh, chronic or subacute conjunctivitis, uveitis, allergic con-junctivitis or surgically treated traumatic detachment of the retina. The rise of int-raocular pressure was reversible in 13 out of 16 eyes by sheer withdrawal of corticosteroid administration. In the remaining 3 eyes an antiglaucomatous operation became necessary to normalize the tension.
