涙小管断裂の縫合法

Repair of severed lower canaliculus

　眼瞼の裂傷に際し，特に内眼角部の裂傷において，涙小管の断裂は日常しばしば経験されることである。このような症例に対し，従来は一次的に皮膚縫合が行われ，その導涙機能に関する考慮が殆んど払われていないことが多かつた。昨年私達は本学会において，涙小管断裂の陳旧例に対し，テトロン製の人工血管を使用して，結膜嚢から鼻腔迄の間に人工的な涙道を形成する方法を発表した。この方法は，断端縫合の不可能な症例にはよい方法であるが，縫合が完全に行えるならばその方が導涙機能の上からもより自然であることは今更論ずる迄もないことである。

　今回私達は過去6カ月の間に，涙小管断裂の陳旧例1例と，新鮮な症例5例を経験し，そのすべてに断端を発見することが出来，両断端の縫合を行つた。縫合後の経過は良好であり，2〜3工夫を必要とする点も考えられるので，以下その断端発見法と，縫合法につき論じてみたい。

Severance of the canaliculus is a frequent complication in laceration of the lid.

In repairing severed canaliculus, we have reestablished normal function by inserting 3-0 chromic Catgut from the punctum thro-ugh the lacrymal sac to the nasal cavity.

In identifying the medial segment of the severed canaliculus, we used three methods. If the upper canaliculus has not been injured, first, sterile milk may be injected into it with the canula, second, 02 curved canaliculus probe may be inserected into it and they will be seen to issue from the lower canaliculus submerged in the tissues.
