涙小管閉塞に対する涙小管涙嚢吻合術について

A New Procedure for the Obstruction of Canaliculus

I．緒言

　涙道の閉塞に基因する流涙症のうち，鼻涙管閉塞および慢性涙嚢炎に対しては，涙嚢鼻腔吻合術が根治的治療法として確立されているが，涙小管閉塞に対しては，現在まだよい方法は見出されていない。そのため，涙小管閉塞による流涙症は難治流涙症として，根治的治療法の開発が期待されている。

　私どもは涙小管閉塞に対して，閉塞部を切除し，涙小管を涙嚢に直接縫合する方法を考案した。この方法で涙小管閉塞10例に手術を行ない，とくに涙小管閉塞のうち，大部分を占める閉塞部位が涙嚢付近にあるものにはすぐれた成績が得られたので，涙小管閉塞に対する根治的治療法として，それを紹介しておきたいと思う。

The obstruction of the lacrimal canaliculus has been one of the most difficult conditions which could be brought to cure through surgical means for the relief of epiphora. A new opera-tive procedure has been practised by the present authors which consists in removal of the obstru-cted portion of the canaliculus and in suturing the patent portion of the canaliculus directly to the lacrimal sac with the catgut passing through the lumen. Ten cases has so far been operated on after this procedure. The outcome was suc-cessful in 6 cases.
