外傷性涙小管断裂の治療

Surgical Repair of Lacerated Lacrimal Canaliculus

緒　言

　眼瞼の裂傷，特に内眼角部近くの眼瞼裂傷では涙小管断裂を伴うことが多く，ことに下涙小管断裂が多く認められる。このような場合，涙小管断裂に気付かれず眼瞼裂傷に対してだけ処置が行なわれると以後の導涙機能は障害され，患者は流涙に悩まされ続けることになる。

　今回著者らは，涙小管断裂を伴う眼瞼裂傷新鮮例を6例経験したので，その手術成績をまとめてここに報告する。

We treated 6 fresh cases of traumatically lacerated lacrimal canaliculus. Five cases were children of 3 years old or less. The lower cana-liculus was injured in 5 cases and the common canaliculus in one case. We could locate the medial end of the severed canaliculus either throughdirect observation with operation micr-oscope (2 cases), injection of gentian violet through the intact upper canaliculus (one case), or by inserting a curved probe which was a makeshift device using ordinary sawing needle (3 cases).
