全層角膜移植における虹彩前癒着の予防並びに処置

Prevention and management of anterior synechiae in keratoplasty

Ⅰ．緒言

　角膜移植後の合併症としてしばしばみられるものの一つとして虹彩前癒着があり，Paton （1957）^15）は術中に虹彩損傷又は出血を起した場合に発生しやすいと述べ，Fine^6）15％，Leahey^12）23％，Alberth^{6〜12％，Doctor5）27％，及Ma—rtin and Smith14）23.7％，の発生率を報じた。}

　この合併症は手術手技の熟達によりその発生率を少なくする事が出来るがその発生を予防する事が第一である。不幸発生した場合には出来るだけ障害を少なくして完全な癒着剥離を行なわねばならない。

A method is presented to prevent anterior synechia formation in keratoplasty. Befor making incision into the recipient eye, Sato's knife is passed obliquely into the anteriorchamber and. is removed. The blade is pre-viously dipped in fluorescein solution and is allowed to dry A clearly defined track is thus produced along the knife, through which air or 3% choddroitin sulfate solution can be injucted into the anterior chamber via 27 gauge needle after the penetrating graft has been fixed. Synechia formation can thus be prevented.
