トリプシンによるチン氏帯離断に関する臨床的研究

Clinical study on enzymatic zonulolysis using Trypsin

I．緒言

　著者はα—chymotrypsin （以後chと略）を用いたEnzymatic Zonulolysisについて一連の基礎的実験や臨床的研究を行いその成績について報告した^1）2）3）。更に同じ蛋白分解酵素であるTrypsin （以後Trpと略）を用いたEnzymaticZonuolysisに関して家兎眼による基礎的実験を行い，その結果を本年の日眼総会に於て発表した^4）。

　それにつづき今回は実際の白内障全摘出術に当つてのTrpの使用基準を確立することを目的としてTrpを用いて行った白内障全摘出術50例の臨床成績について述べ，更にこれをChによる100例及びこれらの酵素を用いないで行った100例の成績と比較し検討を加える。

Trypsin was used as a zonulolytic agent for 50 cases of the total extraction of cataract, and the results were compared with those of cases operated with α-chymotrypsin and of cases without enzymatic zonulolysis.

1. Trypsin solution of various concentrantions and application time (Table 3) was used for the total extraction (Operative techniques in Table 4). It was found that trypsin had as marked zonulolytic effect as α-chymotrypsin, and the delivery of the lens was very much facilitated.

2. In the incidence of the ruptured capsule during the course of the operation no signifi-cant difference was found among three groups.
