Paecilomyces lilacinusによる角膜真菌症の1例

Oculomycosis caused by paecilomyces lilacinus

　Paecilomyces lilacinusによる角膜真菌症の1例を報告する。症例は全身的には糖尿病を合併し，局所的には角膜ヘルペスでステロイドの局所投与を頻回に行っていた。

　角膜所見は固くもり上り表面の乾いた病巣で進行は遅く，抗真菌剤はほとんど無効でチメロサールが有効であった。

　分離した真菌を家兎角膜に接種した結果，数日後に潰瘍が発生したが経過観察中に自然治癒した。またステロイド投与例では，非投与例に比べて症状は重篤で治癒が著しく遅れた。

A 70-year-old male developed mycotic infection of the cornea due to paecilomyces lilacinus in his right eye. He had been suffering from diabetes mel-litus for the pase 10 years. Purulent keratitis devel-oped in the left eye one year before and was cured by antibiotics therapy. Recurrent herpetic kerati-titis was present during the past year and was treat-ed by topical corticosteroids.

Initially, a small ulcer appeared in the right cornea. The ulcer became larger and was accom-panied by hypopyon one month later.
