We evaluated 3 pieces of angle-supported ante-rior chamber lens which were removed from the eye due to therapeutic reasons. The optics and haptics were made of polymethylmetacrylate (PMMA). With the use of transmission electron microscopy, we compared the cellular reaction on the optical portion with that on the haptics.

We observed fibrovascular proliferation, infiltra-tion of multinucleated giant cells, macrophages and fibroblasts around the haptics. The finding was compatible with that of typical foreign body granuloma. The foreign body granuloma was con-sistently absent around the optics. The lack of foreign body reaction appeared to be because the optics was in touch with the aqueous humor only and not with the iris stroma. The findings also seemed to explain why the optics of intraocular lenses remain transparent when properly placed.
