　3症例より摘出された前房レンズ周囲の生体反応を，主に透過型電子顕微鏡を用いて検討した。これらのレンズは光学部，支持ループともポリメチルメタクリレート（PMMA）からできており，光学部のあった前房内とループが接していた虹彩のPMMAに対する反応の差を観察した。

　支持ループ周囲の虹彩には，血管結合織の増生，多核巨細胞，マクロファージ，線維芽細胞などの浸潤があり，いわゆる異物肉芽腫の形成があったが，前房内にある光学部周囲には一部に多核巨細胞があったものの，典型的な異物肉芽腫の形成はなかった。このように，前房内あるいは後房内では，PMMAに対する典型的な異物肉芽腫の形成が起こらないので，眼内レンズは透明性を保つことができると考えられた。

We evaluated 3 pieces of angle-supported ante-rior chamber lens which were removed from the eye due to therapeutic reasons. The optics and haptics were made of polymethylmetacrylate (PMMA). With the use of transmission electron microscopy, we compared the cellular reaction on the optical portion with that on the haptics.

We observed fibrovascular proliferation, infiltra-tion of multinucleated giant cells, macrophages and fibroblasts around the haptics. The finding was compatible with that of typical foreign body granuloma. The foreign body granuloma was con-sistently absent around the optics. The lack of foreign body reaction appeared to be because the optics was in touch with the aqueous humor only and not with the iris stroma. The findings also seemed to explain why the optics of intraocular lenses remain transparent when properly placed.
