レンズ交換を要したハイドロジェル眼内レンズの1例

A case of hydrogel intraocular lens replacement for lens opacification

67歳女性が左眼に白内障手術と眼内レンズ挿入を受けた。術後に0.7の矯正視力が得られたが，半年後から眼内レンズが混濁しはじめた。20か月後に視力が0.2になり，眼内レンズの摘出と挿入を行った。術中に合併症があり，前部硝子体切除とレンズの毛様溝縫着を必要とした。以後の経過は順調で，視力は0.5で安定し，以後の1年間はレンズの再混濁はない。患者が当初に用いたのはハイドロジェル眼内レンズであった。製品が改良された現在では混濁発生の可能性は小さいが，再手術では手術の難易度を考慮する必要がある。

A 67-year-old woman received cataract surgery with implantation of intraocular lens（IOL）in her left eye. She regained visual acuity of 0.7. The implanted hydrogel IOL started to opacify 6 months later. She received extraction and reinsertion of IOL 20 months after initial surgery. Anterior vitrectomy and sulcus suture had to be performed due to intraoperative complications. She has been doing well thereafter with visual acuity of 0.5 and with no opacification of IOL. This case illustrates that due precaution is necessary in extraction and reinsertion of hydrogel IOL.
