在胎34週出生児にみられたaggressive posterior ROPの1例

Aggressive posterior retinopathy of prematuriy in an infant born after gestation of 34 weeks

要約　目的：Aggressive posterior retinopathy of prematurityが発症した在胎34週で出生した極低出生体重児の報告。症例：在胎34週0日の女児で，生下時体重は1,246gであった。高濃度酸素を10日間継続使用し，日齢18で眼科を受診した。所見：黄斑部近くまで無血管野があり，zone Ⅰに相当した。眼底後極部の網膜血管は強く拡張蛇行し，円周方向に環状に走る血管吻合があり，aggressive posterior retinopathy of prematurityと診断し，翌日にレーザーによる汎網膜光凝固を行った。網膜剝離と牽引乳頭は生じなかった。結論：新生児では，在胎週数や生下時体重に関係なく，aggressive posterior retinopathy of prematurityが発症することがある。

Abstract.　Purpose：To report an infant who was born after gestation of 34 weeks and who developed aggressive posterior retinopathy of prematurity. Case：A female infant was born after gestation of 34 weeks weighing 1,246 g. She received high-concentration oxygen for 10 days. She was referred to us at the age of 18 days. Findings：Both eyes showed avascular retinal area in the vicinity of the macular area. Vessels in the posterior retina showed marked tortuosity and dilatation. Anastomoses were present in the circular direction. The findings were those of aggressive posterior retinopathy of prematurity. Both eyes received laser panretinal photocoagulation the following day. Retinopathy of prematurity subsided leaving no retinal detachment or dragged disc. Conclusion：This case illustrates aggressive posterior retinopathy of prematurity may develop regardless of birthweight or gestational age.
