Abstract. Purpose:To report changes in refraction in 571 eyes over a period of 30 years or longer. Cases and Method:This retrospective study was made on 571 eyes of 294 persons followed up in an eye clinic for 30 years or longer. Cases were excluded including cataract, glaucoma or high astigmatism. All the eyes were examined using a refractometer under cycloplegia. The age at initial examination ranged from one to 59 years. A total of 240 eyes(42%)were under 18 years of age and 156 eyes(27%)were over 40 years of age. Cases were divided into 7 groups according to the initial age for analysis. Results:Refraction shifted towards myopia in 217 out of 240 eyes(90%)who were 18 years or less initially. Refraction shifted towards hyperopia in 99 out of 156 eyes(63%)who were 40 years or older initially, remained unchanged in 40 eyes(26%),and shifted towards myopia in 17 eyes(11%). Conclusion:After follow-up for 30 years or longer, tendency towards myopia was present in persons aged 18 years or younger initially. Tendency towards hyperopia became apparent in persons aged 30 years or more initially.
