アレルギー性結膜疾患簡易迅速診断キット「アレルウォッチ涙液IgE」の臨床的検討

Clinical appraisal of Allerwatch Tear Fluid IgE,a diagnostic kit for allergic conjunctival diseases

要約　目的：アレルギー性結膜疾患で涙液の総IgEを測定するアレルウォッチ涙液IgEと海外類似キットの多施設による評価と比較。対象と方法：本研究は5施設で得られたアレルギー性結膜疾患109検体と対照47検体について行われた。疾患の内訳は，季節性アレルギー性結膜炎65例，通年性アレルギー性結膜炎34例，アトピー性角結膜炎6例，春季カタル4例である。アレルウォッチ涙液IgEと海外で製造された類似キットで涙液総IgEを測定し，臨床診断と比較した。結果：臨床診断との一致率は，アレルウォッチ涙液IgEが海外類似キットよりも有意に高かった。特異度（陰性一致率）はいずれも100％であった。結論：アレルウォッチ涙液IgEによる涙液の総IgEの測定は，アレルギー性結膜疾患の診断に有効であり，輸入された海外類似キットよりも高い診断一致率を示した。

Abstract.　Purpose：To clinically evaluate the value of Allerwatch Tear Fluid IgE and an imported diagnostic kit in the differential diagnosis of allergic conjunctival diseases. Cases and Method：This study was made by five institutions on samples of tear fluid from 109 cases of allergic conjunctival disorders and 47 controls. The cases comprised seasonal allergic conjunctivitis 65 eyes,perennial allergic conjunctivitis 34 eyes,atopic keratoconjunctivitis 6 eyes,and vernal conjunctivitis 4 eyes. The tear fluid from patients and controls were tested by Allerwatch Tear Fluid IgE and an imported diagnostic kit. The findings were compared with the clinical diagnosis. Results：Allerwatch Tear Fluid IgE showed a significantly higher rate of concordance with the clinical diagnosis than the imported diagnostic kit. Both kits showed 100％ of rate of concordance in specificity. Conclusion：Allerwatch Tear Fluid IgE was useful in the supplementary diagnosis of allergic conjunctival diseases. It showed a higher rate of concordance with the clinical diagnosis than the imported diagnostic kit.
