I．はじめに

　内頸・眼動脈分岐部動脈瘤（以下IC-Oph An）は蝶形骨，視神経および視交叉，海綿静脈洞に近接して発生するが，その解剖学的特殊性から直達手術が困難な場合が多く，手術成績も他の部位の動脈瘤に比して満足すべきものではない^1-3,10）．最近，本動脈瘤に対する手術法および手術手技^{6,11,13,17）}，接近法^9,12,14）に関しての報告は比較的多くみられるが，動脈瘤の圧迫や手術操作による手術前後の視機能障害についての検討も欠かせないものと考えられる．

　今回われわれは現在までに経験したIC-Oph An症例を対象として，本動脈瘤に伴う視力，視野障害について，SAHの有無，放射線学的所見および手術所見より分析，検討した．

The direct neck clipping for carotid-ophthalmic aneurysm is hazardous and difficult as compared with other intracranial aneurysms because of its location adjacent to the anterior clinoid process, the optic nerve, and the cavernous sinus.

The authors discuss the visual abnormalities ac-companied by carotid-ophthalmic aneurysm on the basis of our experiences with 30 patients with 33 aneurysms. Ten patients (4 of 14 with ruptured aneurysms, 6 of 16 intact aneurysms) had preopera-tive visual abnormalities. Of our series of patients, visal acuity and visual field were impaired in one eye.
