周辺部網膜出血を認めたC型肝炎ウイルス関連網膜症の1症例

Peripheral retinal hemorrhage in a case of hepatitis C virus-associated retinopathy

要約　目的：網膜周辺部に出血が生じたC型肝炎ウイルス網膜症の1症例の報告。症例：61歳男性がインターフェロン投与前の眼科的検査のため紹介受診した。43年前に腹膜炎の手術の際に輸血を受けた。11年前にC型肝炎が発見され，1年前に肝細胞癌に対して肝切除を受けた。所見：矯正視力は右1.2，左1.0で，両眼底の耳側周辺部に毛細血管瘤と出血斑が多発していた。インターフェロン投与は行わなかった。1年間の蛍光眼底造影による経過観察で，耳側周辺部の無灌流領域が拡大した。結論：インターフェロン網膜症では網膜出血と軟性白斑が生じることがある。本症例では眼底周辺部に無灌流領域と毛細血管瘤が主要な病変であった。

Abstract.　Purpose：To report a case with hepatitis C with peripheral retinal hemorrhage as the major ocular manifestation. Case：A 61-year-old male was referred to us regarding initiation of interferon treatment for hepatitis C. He had received blood transfusion during abdominal surgery 43 years before. Hepatitis C was detected 11 years before. He had received hepatectomy one year before for liver carcinoma. Findings：Corrected visual acuity was 1.2 right and 1.0 left. Both eyes showed capillary microaneurysms and hemorrhagic patches in the temporal periphery. Interferon treatment was withheld. During one year of observation, fluorescein angiography showed enlargement in the area of retinal nonperfusion in the temporal periphery. Conclusion：This case illustrates that nonperfusion with capillary microaneurysms may be major manifestations of hepatitis C virus retinopathy.
