Abstract. Purpose:To report a case of age-related macular degeneration treated by vitrectomy and artificial giant retinal tear to remove subretinal hematoma and fibrotic mass. Case:A 61-year-old male had been diagnosed with bilateral age-related macular degeneration 33 months before. His left eye received two sessions of photodynamic therapy. His corrected visual acuity was 0.1 right and 0.06 left. The left eye showed subretinal hematoma with bullous retinal detachment. He received vitrectomy and artificial giant retinal tear to remove the hematoma and fibrotic mass that apparently had formed by photodynamic therapy and that showed firm adhesion to the retinal pigment epithelium(RPE). He has been doing well for 6 months until present with visual acuity of 0.04. Conclusion:This case illustrates that firm adhesion may form between fibrotic mass and RPE following photodynamic therapy for age-related macular degeneration. Vitrectomy and artificial giant tear may be useful for this condition.
