Abstract. A 58-year-old female was involved in a head-on crush while driving a light truck. She was hit by a protruding object in the cabin on her face. When seen immediately after the accident, her corrected visual acuity was no light perception right and 1.2 left. Her right eye showed circumferential rupture of bulber conjunctiva, hyphema and prolapse of eyeglobe anterior to the eyelids. Computerized tomography showed ruptured optic nerve and no fracture of bony orbit. The prolapsed eyeglobe was repositioned under general anesthesia. Fourteen months after injury, the eyeglobe is well preserved with fair motility. The patient is satisfied with the cosmetic outcome. This case illustrates that repositioning of eyeglobe can be the treatment of choice whenever feasible.
