眼科受診を契機として判明した慢性硬膜下血腫の1例

Ophthalmological consultation led to the detection of subdural hematoma.Report of a case

55歳男性が飛蚊症で某医を受診し，両眼の乳頭浮腫が発見され，紹介された。血小板減少の既往と睡眠時無呼吸症候群がある。矯正視力は左右とも1.2であった。両眼に乳頭浮腫，左眼に軽度の硝子体出血があった。左眼にマリオット盲点の拡大があり，頭部磁気共鳴画像検査（MRI）で左側の慢性硬膜下血腫が発見された。穿頭洗浄術で血腫が除去され，約2か月後に両眼の乳頭浮腫は著しく改善した。

A 55-year-old male sought medical advice for myodesopsia and was diagnosed with bilateral papilledema in another institution. He had past history of thrombocytopenia and apnea syndrome during sleeping. When seen by us, his corrected visual acuity was 1.2 in either eye. Papilledema was present in both eyes. The left eye showed slight vitreous hemorrhage and enlarged blind spot. Magnetic resonance imaging（MRI）showed chronic subdural hematoma in the left hemisphere. Drainage of hematoma was followed by remarkable improvement of bilateral papilledema 2 months later.
