Abstract. Purpose:To report the outcome of primary penetrating keratoplasty. Cases and Method:This retrospective study was made on 53 eyes of 48 patients who received penetrating keratoplasty at Kaiya Eye Clinic during the past 52 months. The series comprised 17 eyes of 17 males and 36 eyes of 31 females. Their age ranged from 20 to 88 years,average 66 years. The causative lesions included corneal leukoma 23 eyes(43%),bullous keratopathy 15 eyes(28%),corneal perforation or ulcer 9 eyes(17%),and 2 eyes each of keratoconus and corneal degeneration(4%). The donor cornea was supplied from USA in 26 instances and by an eye bank in Japan in 27 instances. Results:Postoperative complications included graft rejection 51%,ocular hypertension 19%,corneal ulcer or perforation 11%,endophthalmitis 9%,bullous keratopathy 8%,and persistent epithelial defect 6%. The corneal graft remained clear in 68% of eyes after one year and in 50% of eyes after 5 years of surgery. Conclusion:Prevention of graft rejection and selection of adequate cases are important in primary penetrating keratoplasty.
