外傷性低眼圧黄斑症の5例

Review of five cases of posttraumatic hypotony maculopathy

要約　鈍的外傷に続発した低眼圧黄斑症の自験例5例5眼の経過を検索した。男性3例，女性2例であり，年齢は31～45歳であった。全例に隅角解離があり，その範囲は30～60°であった。2例では眼圧が特発的に正常化し，黄斑症は消失した。低眼圧の持続期間が長い1例と，隅角解離の広い2例には手術が奏効した。うち2例には，隅角解離がある部位に強角膜切開を行い，虹彩根部を10-0ナイロン糸で隅角に連続縫合して周辺虹彩前癒着を作製する新術式を用いた。この隅角連続縫着術は手技が簡単であり，特に広範囲の隅角解離に有効であると考えられる。

Abstract.　We reviewed the course of 5 eyes in 5 patients of hypotony maculopathy following blunt ocular trauma. The series comprised 3males and 2 females. Their age ranged from 31 to 45 years. All the affected eyes had cyclodialysis extending from 30 to 60 degrees. In 2 eyes,maculopathy disappeared following spontaneous normalization of intraocular pressure. Surgery was performed on one eye with longstanding hypotony and on 2 eyes with extensive cyclodialysis. In 2 eyes,cyclodialysis was closed by fixating the iris root to the chamber angle with a running suture of 10-0 nylon thread. All the operated 3 eyes recovered normal IOP and visual acuity. Our technique of fixating the iris root to the chamber angle with a running suture is easy to perform and is indicated for cyclodialysis of wide extent.
