外傷性低眼圧黄斑症に対する手術治療

Surgical treatment in a case of traumatic hypotony maculopathy

　56歳男性にみられた外傷性毛様体解離による低眼圧黄斑症の1例を経験した。受傷後，約12週間で低眼圧黄斑症をきたした。約30°の毛様体解離を認め，保存的に経過をみたが，視力低下をきたした発症後4か月めにレーザー光凝固術を2度行った。処置後眼圧は上昇したが，数日で低眼圧となったためジアテルミー凝固術と毛様体解離部直接縫合術を併用して行った。翌日から眼圧は正常化し前房は深くなったが，新たに約30°の毛様体解離を発見した。3か月間経過を観察したが視力低下が進行したため，再度新たな毛様体解離に対して同様の手術を行い，低眼圧黄斑症は軽快した。低眼圧黄斑症の治療は，毛様体解離を完全に閉塞することが大切であると考えられた。

A 56-year-old male developed hypotony maculopathy 12 weeks after blunt ocular trauma.Anterior cyclodialysis was present over 30 degrees of the circumference. Photocoagulation was repeated twice but the intraocular pressure rose only temporarily. We then performed diathermic coagulation with suturing of the dialyzed ciliary body to the sclera. We then detected anterior cyclodialysis in a previously unsuspected sector.Suturing of the ciliary body 3 months later normal-ized the intraocular pressure with disappearance of hypotony maculopathy.
