Abstract. A 72-year-old woman presented with blurring of her right eye since 3 days before. She had had severe headache during the last 16 months. Her left visual acuity had started to deteriorate 8 months before to result in blindness 5 months later. Her corrected visual acuity was now 0.2 right and no light perception left. We diagnosed her with ischemic optic neuropathy. Her right visual acuity suddenly decreased to 0.03 5 months later. Gadolinium-enhanced magnetic resonance imaging(Gd-MRI)showed hypertrophic pachymeningitis as the presumed cause of optic neuropathy. Pulsed corticosteroid therapy resulted in reduced thickness of the dura mater and in improvement of visual acuity to 1.0. This case illustrates that hypertrophic pachymeningitis may induce optic neuropathy.
