網膜細動脈瘤を合併したサルコイドーシスの4例

Retinal arteriolar macroaneurysm in four cases of sarcoidosis

要約　眼サルコイドーシスとして治療または観察中の4例に網膜細動脈瘤が発生した。細動脈瘤は3例が片眼性，1例が両眼性であった。すべて女性で，年齢は51，54，62，66歳であり，細動脈瘤の好発年齢よりも若かった。全例に高血圧などの動脈硬化性疾患があり，3例に重篤な心疾患があった。細動脈瘤はぶどう膜炎が鎮静化した時期に出現した。治療として3眼に光凝固を行った。動脈硬化性疾患がある眼サルコイドーシスでは，炎症の鎮静期に網膜細動脈瘤が発生する可能性があるので注意を要する。

Abstract.　We observed four patients who developed retinal arteriolar macroaneurysm during treatment of ocular sarcoidosis. Macroaneurysm was unilateral in 3 cases and bilateral in one. All were females and were aged 51,54,62,66 years respectively. All the four cases had systemic hypertension and arteriosclerosis. Severe cardiovascular disease was present in 3 cases. Macroaneurysm developed after the active uveitis had subsided. Photocoagulation was performed in 3 eyes followed by prompt occlusion of macroaneurysm. These cases illustrate that macroaneurysm may develop during the quiescent stage of ocular sarcoidosis,particularly when systemic arteriosclerosis is present.
