急性前部ぶどう膜炎94症例の臨床的検討

Clinical features of 94 cases of acute anterior uveitis

（B3-2-2）　東京医科大学眼科で過去13年間に急性前部ぶどう膜炎と診断された94症例，121眼を対象として，その臨床所見，経過，予後などについてレトロスペクティブに検討を行った。その結果眼所見については従来の報告とほぼ同様であったが，螢光眼底撮影にて異常所見を示す症例が約半数例に及んだ。また，一年以上経過を観察できた症例の半数以上に炎症の再燃がみられた。HLA-B27の有無による臨床像を比較検討したところ，眼所見にはほとんど差異をみなかったが，炎症再燃例はHLA-B27陽性例に有意に多い結果となった。また，全94例中，潰瘍性大腸炎や強直性脊椎炎などの基礎疾患を有するものが17例存在していた。急性前部ぶどう膜炎という疾患単位には，さまざまな要因がその発症に関与していることが示された。

We reviewed 121 eyes of 94 patients with acute anterior uveitis seen during the foregoing 13 years. Clinical manifestations were almost the same as in previous reports. Fluorescein angiography showed abnormal findings in about 50% of the cases. Recurrence developed in about 50% of cases who could be followed up for one year or longer. Cases with HLA-B27 showed similar clinical features as in those without but developed more frequent recurrences. Systemic diseases including ulcerative colitis and ankylosing spondylitis were present in 17 patients (18%). The findings show the multiplicity in the etiology of acute anterior uveitis.
