A 29-year-old male was diagnosed as sarcoidosis with a large optic disc granuloma in his right eye. The granuloma continued to enlarge after treatment by topical and systemic corticosteroid. Subsequent exudative retinal detachment led to deterioration of visual acuity to hand motion. Vitrectomy was performed by removal of the vitreous gel and irrigation by fluid containing betamethasone. After drainage of subretinal fluid, betamethasone was injected into the granuloma. Postoperative course was favorable. The visual acuity improved to 0.6 with no recurrence of inflammation except anterior uveitis. This case illustrates the usefulness of vitrectomy with injection of corticosteroid into the granuloma in sarcoidosis of the optic disc.
