A 68-year-old man presented with blurring of vision in his left eye since 3 months before. He had received surgery for advanced stomach cancer followed by chemotherapy 2 years before. His corrected visual acuity was 1.2 right and 0.3 left. His left eye showed disc swelling, macular edema, and vitreous opac-ity. Atrophic pigmented scars were present in the nasal periphery with an exudative lesion 3 disc diameters across. Fluorescein angiography showed findings suggesttive of retinitis. Cut-off index for IgM levels of toxo-plasma antigens was high at 1.8. The toxoplasma antibody titer rose from 1024 to 4096 after systemic acetyl-spiramycin. Systemic treatment with prednisolone was followed by inactivation of fundus lesions and im-proved visual acuity to 0.8. It appeared that he had been exposed to toxoplsma during the period of immunop-pression and that ocular involvement had become manifest after optic papillitis.
