Abstract. We treated 3 eyes of 3 patients of subfoveal polypoidal choroidal vasculopathy by transpupillary thermotherapy(TTT)at the same setting. All were males and were aged 68,76 and 79 years. All the eyes showed similar clinical findings. Each eye had serous retinal detachment in the fovea. The visual acuity was 0.5 in each. Fluorescein angiography showed occult choroidal neovascularization. Indocyanine green angiograpahy showed occult choroidal vessels. A diode laser was used in TTT. The diameter was set at 3 mm. A single shot was applied at 350 mW and for 60 seconds. After treatment, retinal detachment disappeared in one eye and the visual acuity improved to 0.9. In another eye,the treated retinal area became opaque and the visual acuity deteriorated to 0.1. In the third eye, retinal detachment increased resulting in macular atrophy with the visual acuity of 0.2. These 3 eyes showed different outcomes following treatment in spite of initially similar fundus findings. We advocate further studies to determine the optimal condition of TTT.
