ベバシズマブ硝子体内投与により退縮した点状脈絡膜内層症に併発した脈絡膜新生血管

Resolution of choroidal neovascularization after intravitreal bevacizumab in three cases of punctate inner choroidopathy

要約　目的：点状内層脈絡膜症（punctate inner choroidopathy）に併発した脈絡膜新生血管（CNV）がベバシズマブの硝子体内投与で退縮した3症例の報告。症例と経過：3症例とも女性で，年齢は24，36，57歳であり，いずれも片眼性の発症であった。2眼には－8D，1眼には－2Dの近視があり，矯正視力はそれぞれ0.5，0.2，0.08であった。眼底後極部に白点が散在し，蛍光眼底造影でⅡ型CNVがあった。CNVを伴った点状内層脈絡膜症と診断した。1～3回のベバシズマブの硝子体内投与を行った。CNVは退縮し，視力はそれぞれ1.2，1.0，0.8に改善した。結論：3例の点状内層脈絡膜症に併発したCNVに対し，ベバシズマブの硝子体内投与が著効を示した。

Abstract.　Purpose：To report 3 cases of punctate inner choroidopathy with choroidal neovascularization（CNV）treated by intravitreal bevacizumab. Cases and Findings：All the 3 cases were females and were aged 24,36 and 57 years respectively. All were unilaterally affected. Refraction was －8 diopters in 2 cases and －2 diopters in one. Corrected visual acuity was 0.5,0.2 and 0.08 respectively. All the affected eyes showed white dots scattered in the macular area. Fluorescein angiography showed type 2 CNV. CNV resolved after one to 3 sessions of intravitreal injection of bevacizumab. Visual acuity improved to 1.2,1.0 and 0.8 respectively. Conclusion：These cases illustrate that intravitreal bevacizumab may be effective for CNV in eyes with punctate inner choroidopathy.
