We identified a peculiar finding,to be called as vermilion polyp,in cases of polypoidal choroidal vasculopathy(PCV)resulting in huge hematoma. This condition differs from orange polypoidal lesion or hemorrhagic detachment of retinal pigment epithelium(RPE)by its brilliant red color. It is characterized by faint granular hyperfluorescence in fluorescein angiogram and by multiple polypoidal lesion with massive leakage beneath detached RPE in indocyanine green angiogram. In a series of 146 eyes of PCV manifesting hematoma,the hematoma was larger than 5 disc diameters in 55 eyes(38%). There were 15 eyes(10%)with vermilion hematoma,of which 11 eyes(73%)had huge hematoma. This finding shows that vermilion polyp in PCV may be an impending sign of huge hematoma.
