A 59-year-old woman presented with blurring of vision in her right eye since one year before. Her corrected visual acuity was 0.5 right and 0.9 left. A retinal angioma of 2 disc diameters was present in the inferior peripheral fundus. It was surrounded by circumscribed retinal detachment. Cystoid macular edema(CME)was present. After direct laser photocoagulation proved futile,transscleral cryocoagulation was applied to the angioma. Retinal detachment and CME still persisted. Another 5months later,transpupillary thermotherapy(TTT)was performed with power output of 500mW,spot size of 3mm,duration of 60 sec and 4 pulses. The treatment was followed by exacerbation of retinal detachment. The visual acuity decreased from 0.2 to 0.06. Scarring of angioma was achieved by 7 sessions of photocoagulation. Retinal detachment and CME disappeared. The final visual acuity was 0.02 due to macular atrophy. It appears that TTT failed to destroy the angioma and induced extravasation from the blood vessels.
